New research suggests that early problems in the brain's waste-clearing system may raise the risk of psychosis. Scientists have linked these disruptions to 22q11.2 deletion syndrome, a genetic condition where up to 40% of affected individuals later develop psychotic symptoms. The findings highlight how brain vulnerabilities can appear years before any signs of illness.

22q11.2 deletion syndrome is a rare genetic disorder that affects roughly one in 4,000 people. Those with the condition face a 20 to 40 times higher risk of schizophrenia and related disorders compared to the general public. Studies from 2016 to 2026 point to disrupted genes, including the COMT gene, reduced grey matter in the prefrontal cortex, and altered dopamine activity as key factors in this heightened risk.

The glymphatic system, which clears waste from the brain, normally becomes more efficient with age. However, in individuals with 22q11.2 deletion who later developed psychosis, this improvement failed to occur. Instead, their brain clearance remained impaired from childhood onward. Researchers believe this long-term weakness may allow inflammation and neuronal damage to build up, increasing susceptibility to symptoms like hallucinations and delusions.

A first psychotic episode often coincides with cognitive decline, suggesting that pre-existing brain vulnerabilities play a critical role. The study's authors stress that these disruptions are present long before any symptoms appear, offering a potential target for early intervention.

The findings connect early glymphatic dysfunction to later psychosis in high-risk individuals. With 0.5 to 3% of the general population experiencing psychotic disorders, this research could help identify those most at risk. Further studies may explore whether supporting brain clearance could reduce the likelihood of developing schizophrenia.
