Bovine colostrum has gained attention as a potential supplement for human neonates, particularly preterm infants. Its rich composition of bioactive compounds, including immunoglobulins and growth factors, offers a promise to address nutritional and protective gaps in donor milk or formula. Research on bovine colostrum has shown its ability to support gut maturation and immunity in animal models. This has spurred interest in its application for human newborns. Early postnatal administration may enhance receptiveness to its bioactive molecules compared to delayed use.

However, clinical trials in preterm infants have not yet delivered the expected improvements in growth, necrotising enterocolitis (NEC) rates, or sepsis prevention. Processing methods like pasteurisation and freeze-drying can also reduce the potency of its key components, potentially limiting their benefits.

Combining bovine colostrum with prebiotics and probiotics might improve its effectiveness by positively influencing the neonatal gut environment and reducing inflammation. Yet, concerns remain about immune compatibility, as bovine immunological components may not perfectly match human neonatal needs, raising the risk of allergenic reactions.

Experts highlight the need for further research to understand how bovine bioactive molecules interact within the human gut. Advanced techniques such as proteomics and metabolomics are required to analyse these components in detail. Multicentre, randomised controlled trials with standardised protocols are also essential to assess its clinical efficacy and long-term neurodevelopmental safety. While bovine colostrum shows potential, its benefits for human neonates are not yet fully confirmed. Standardised trials and deeper molecular analysis are necessary to determine its true efficacy and safety. Without these, its role in neonatal care remains uncertain.
