FDA fast-tracks Genentech's Enspryng® as breakthrough thyroid eye disease treatment

Genentech has announced a major step for thyroid eye disease (TED) treatment. The US Food and Drug Administration (FDA) has accepted a priority review for Enspryng® (satralizumab) as a potential therapy. TED is a rare but serious condition affecting roughly 155 in every 100,000 people and can cause severe vision problems and disfigurement. The FDA’s decision follows positive results from two global Phase III trials, SatraGO-1 and SatraGO-2. These studies tested Enspryng’s safety and effectiveness in patients with moderate to severe TED. No new safety concerns appeared, aligning with its established profile in treating neuromyelitis optica spectrum disorder (NMOSD).

In SatraGO-1, 49% of patients on Enspryng saw improvement in proptosis (bulging eyes), compared to 31% on placebo. In SatraGO-2, the figure rose to 53%, against 23% for placebo. The drug also reduced double vision for 44% to 61% of patients across the two studies. Additionally, 78% to 90% of participants experienced a drop in clinical activity score (CAS).

Enspryng is a humanised monoclonal antibody targeting interleukin-6 (IL-6). It is already approved as the first and only IL-6 inhibitor for NMOSD. The FDA is set to decide on approval by October 15, 2026. If successful, Enspryng could become a new option for TED patients. The treatment has shown clear benefits in reducing symptoms and improving quality of life in clinical trials.
