The research involved elderly patients scheduled for laparoscopic gastrointestinal surgery, grouped by their frailty status using recognised assessment tools. A modified Dixon-based sequential allocation method was used to determine the median effective dose (ED50) of nalbuphine for each group. This approach helped identify precise dosing requirements for frail versus non-frail patients.

Results showed clear differences in how much nalbuphine was needed for effective pain relief between the two groups. Frail patients required adjusted doses compared to their non-frail counterparts, reinforcing the importance of frailty assessments in pain management planning. Nalbuphine, a mixed opioid agonist-antagonist, was chosen for its suitability in elderly care due to its distinct pharmacological properties.
