Germany's nitrendipine shortage leaves hypertension patients scrambling for alternatives

Patients in Germany relying on nitrendipine, a widely used calcium channel blocker, have faced ongoing supply issues since last summer. The shortages now affect all major manufacturers, leaving both 10 mg and 20 mg formulations unavailable across the country. Regulators have flagged the problem as part of broader drug supply bottlenecks affecting older medications.

The shortages began in mid-2023, with Aristo reporting production problems for its nitrendipine 20 mg tablets. Initially expected to resolve by the end of that year, the issue has since expanded. By March 2026, three key manufacturers—ratiopharm, Hexal, and 1A Pharma—remain unable to meet demand.

Germany's Federal Institute for Drugs and Medical Devices (BfArM) has officially listed nitrendipine among its reported drug shortages. Some relief is on the horizon, though: ratiopharm and Aliud Pharma plan to resume 20 mg supply by late March, while AbZ expects deliveries to restart by the end of May. Calcium channel blockers like nitrendipine work by limiting calcium entry into cells, helping to keep arteries relaxed and blood pressure controlled. They fall into three main types: nifedipine, verapamil, and diltiazem. However, they are not suitable for patients with heart failure or those recovering from a heart attack. Common side effects include low blood pressure, headaches, flushing, and irregular heartbeats. With nitrendipine in short supply, pharmacists have turned to alternatives like amlodipine and lercanidipine. These substitutes are now more frequently prescribed to manage high blood pressure and related conditions.

The prolonged shortage has forced healthcare providers to adjust treatment plans for affected patients. While some manufacturers anticipate restocking in the coming months, the situation highlights wider challenges in securing stable supplies of essential medications. Authorities continue to monitor the issue as part of broader efforts to address drug availability in Germany.
