Germany sees alarming 79% spike in medication errors in 2025

The German Pharmacists' Drug Commission (AMK) recorded a sharp rise in medication errors last year. Reports jumped from 348 in 2024 to 624 in 2025. Meanwhile, the organisation handled over 11,000 notifications about potential drug risks during the same period.

In 2025, the AMK processed 11,154 alerts regarding possible drug-related issues. Among these, suspected cases of misuse fell slightly, but medication errors surged. The commission also issued 263 risk notifications, including 23 formal letters to healthcare professionals and more than 150 batch-specific recalls or reviews.

Quality problems accounted for over two-thirds of the reported cases. Adverse drug reactions made up the remaining third. However, no details were released on which specific drugs or manufacturers were most frequently linked to defects.

The increase in medication errors marked a significant change from previous years. Despite this rise, the AMK continued its routine monitoring of drug safety across Germany.

The AMK's 2025 report highlights a growing number of medication errors alongside a high volume of risk notifications. While misuse cases declined, quality concerns remained the dominant issue. The organisation took action through recalls, reviews, and direct communication with medical professionals.
