Russia's new traffic laws enforce mandatory medical checks for drivers by 2027

New traffic laws in Russia will soon enforce stricter medical checks for drivers. From March 1, 2027, those who refuse unscheduled health assessments will automatically lose their driving licenses. The changes follow a law passed in July 2025, aiming to improve road safety through tighter medical oversight. The updated rules require drivers to complete an unscheduled medical examination within three months of being notified. Failure to comply will result in an immediate license revocation. Once revoked, the only way to regain driving privileges is by passing the required medical evaluation.

Refusing the check is now treated the same as having a confirmed medical contraindication. If a driver is diagnosed with a condition that impairs their ability to drive—such as certain mental health disorders or vision problems—both the driver and the Ministry of Internal Affairs (MVD) will receive official notification. Medical results will be recorded digitally in the federal registry of the Unified State Healthcare Information System (EGISZ), with conclusions signed using a qualified digital signature. Separately, the State Duma has also advanced a bill in its first reading. This proposal would treat impairment from certain medications as equivalent to alcohol or drug intoxication while driving. However, the final list of banned medications and the penalties for violations have not yet been confirmed.

The law tightens medical requirements for drivers, with revocations starting in early 2027. Those affected will need to pass mandatory health checks to keep or restore their licenses. Officials have yet to finalise details on medication restrictions and related penalties.
